In this example my clients wanted a way for prospective patients to choose a condition they were experiencing and have a personalized explanation sent to the patients email. This was originaly created using Perl, then later updated to PHP 5.

This is a simple refer a friend script I created with PHP 5. Simply fill out all fields and an email will be sent to the person you specify with a link back to the page you came from as well as a personal message of your choice.

Looks like the last example doesn't it? This is example takes the refer a friend program and modifies it to send HTML email rather then plain text.
